Fairfield Community Health Center is hiring a full-time Community Resource Coordinator. This position requires a caring, pleasant personality with the ability to accept the values and lifestyles of patients. A background in social services is required. Duties include coordinating prescription deliveries from our in-house pharmacy, completing prescription assistance program applications, assisting with Senior Health Insurance program applications, assisting with ACA insurance programs, enrolling patients in Medicaid & Medicare and managing patient enrollment in our sliding fee program based on set criteria.
Benefits include vacation days, paid holidays and sick days as well as a quarterly incentive program. Long-term disability insurance and a $50,000 life insurance policy is provided as a benefit at no cost. Optional insurances include: health, dental, vision, accident, critical illness, short-term disability and voluntary life. Two percent match available in the retirement plan.
