Chronic Care Staffing nurses are specifically trained to work with patients to help manage their chronic conditions and coach healthier lifestyle choices as directed by the comprehensive care plan created by their physician. As we help implement Medicare’s Care Management Services for our clients, we complete our mission, and that results in healthier and more satisfied patients.
Job Duties
Care Coordinators call each of their patients once a month to cover the following:
· Chart-review
· Med reconciliation
· Referral coordination
· Prescription refill requests
· Other activities designed to close gaps in care
· Chart/document monthly conversations in the practice's EMR
· Care Coordinators are expected to work from home efficiently to meet daily caseload.
· Develop long-term relationships with patients and improve their quality of life
Care Coordinators are also responsible for conducting verbal enrollments to enroll eligible patients in the program.
Requirements
· Active LPN license
· Active BLS
· 3 years working in a geriatric or family practice setting
· 3 years of EMR charting experience
· Must be able to work 40 hours a week, speaking with patients over the phone, and documenting on your computer for 8 hours a day. This is a fast-paced position; however, you are dedicated to your computer screen for most of your 8-hour shift.
· Must have a home office or HIPPA compliant workspace, someplace secure where you have complete privacy to protect PHI.
· High-speed internet, a computer 3 years or newer (no Chromebooks or MAC PC), and a second monitor are all required to perform the job.
Preferred
· Chronic Care Management (CCM) experience
· Remote work experience
· Remote patient monitoring
