Become a team member of a prestigious residential Substance Use Disorder Treatment Program for women. Chrysalis House is currently hiring a Case Manager to join a multidisciplinary treatment team led by the Chrysalis House Medical Director, an Addiction Medicine Specialist. Chrysalis House has three long-term residential treatment facilities in Lexington, KY for pregnant and parenting women where mothers can heal from past trauma, learn parenting, coping, and life skills, and recover from SUD while staying united with their children.
To increase the likelihood of long-term recovery, Chrysalis House’s model includes the framework of a recovery-oriented system of care in which the focus during treatment is on ongoing chronic disease management rather than short-term stabilization of symptoms.
Chrysalis House is an outcome-driven treatment program, where lengths of stay are determined by tracking severity, function, and progress on treatment goals, rather than a fixed length of stay based on a program model or patient billable days. Federal, state, local, and foundation grants and private donations are utilized to ensure patients can remain in treatment until the targeted level of functional improvement is met.

The case manager position, using discretion and independent judgement, is responsible for coordinating the delivery of services to patients. This position is responsible for conducting and or/monitoring the case management assessment, the treatment planning process and follow-up, preparing written reports, notes and discharge planning. In addition, the case manager provides referral source information, arranges for referrals to appropriate service providers, and ensures access to services. The case manager provides crisis intervention, conducts home visits, and transports patients in company vehicles. This position requires skills in setting short- and long-term goals related to substance abuse and mental health treatment issues. The case manager may also assist with psycho-educational groups and serve as an advocate for clients’ rights and benefits.
Required Education and Experience:
1. Bachelor’s Degree in a human services field from an accredited college or university.
2. Minimum of one year of full-time employment working directly with individuals in a human services setting post completion of a bachelor’s degree.

Licensure/Certification:
