Overview
The Care Manager Supervisor manages a team of Patient Health Navigators who provide comprehensive assessment and evaluation, care planning and coordination, outreach and crisis intervention, linkage to medical, financial, social, rehabilitation and psychiatric services, advocacy, development of social supports and procurement of needed resources. The Care Manager Supervisor monitors and assures performance of the PHN team in the areas above and assures that team/program outcomes are met.

Primary Responsibilities
- Overseeing the engagement activities, comprehensive assessment and service planning the team provides to consumers who have chronic medical problems or significant mental illness.
- Providing weekly/biweekly supervision of team members and facilitating weekly/biweekly team meetings.
- Accompanying team members on planned and unplanned home visits.
- Providing field supervision as needed.
- Completing performance evaluations on team members.
- Submitting disciplinary action and/or corrective action plans for team members where appropriate.
- Participating in management and systems training and development as required.
- Providing “back up” to PHNs who are on-call during evenings/weekends/Holidays.
- Assuring that Agency/Program Safety protocols are adhered to by team members.
- Participating in weekly Management meetings and other Agency meetings as assigned.
- Assuring that team members are successfully managing caseloads, providing care management, and following care plans
- Assuring that PHNs develop, monitor, and periodically update a comprehensive, individualized, consumer centered Plan of Care
- Completing full caseload audits for outgoing team members prior to case transfers to newly assigned PHNs.
- Providing ongoing chart audits of team members.
- Assuring that team members input and retrieve data from various systems including database and web-based systems.
- Assuring that team members respond on a timely basis to all communication related to consumers, internal correspondence, or outside communications.
Qualifications
- Case Management Certification preferred.
- NYS Licensed RN preferred, BSN preferred.
- Experience supervising staff providing care to persons with mental illness preferred.
- Ability to work independently and initiate change
- Have strong organizational and time management skills
- Have strong data management and analysis skills
- Have superior interpersonal skills in collaborating with other providers and consumer centered community support systems

Compensation: $80,000 - $90,000
