We have an exciting new opportunity for a full-time licensed Family Specialist in our Assertive Community Treatment (ACT) program in Tarrytown, NY, and surrounding communities. 

Base Annual Salary Range: $45,000-$50,000.  

SUMMARY of DUTIES:
The ACT team provides non-traditional community-based mental health treatment to promote the recovery and community integration of 68 adults with serious mental illnesses and co-occurring substance use disorders to reduce the use of ERs and inpatient hospital stays. The specialist delivers routine treatment services and is responsible for integrating family goals, assisting with all team members' tasks, and providing family psycho-education. The specialist supports consumers in achieving wellness, recovery, and positive relationships in the community. The Family Specialist will support recovery by providing services that reflect our deeply held values, including recognizing individual rights of self-determination, choice, shared decision-making, and collaboration. When possible, utilize evidence-based practices, always embracing a trauma-informed and person-centered approach.

SPECIFIC DUTIES and RESPONSIBILITIES: 
1.    Conduct a comprehensive assessment of psychiatric history (e.g., onset, course, and effect of illness, past treatment and responses, and risk behaviors), mental status and diagnosis, physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living (e.g., self-care, living situation, nutrition, money management); and family structure and relationships.
2.    Assess and provide services to consumers to address housing, income support, education, vocational training, social support, employment, and primary care needs.  
3.    Involve families and social support in the consumer’s treatment with the team
4.    Provide treatment services, including engagement, problem-solving, wellness self-management, medication support, family support and treatment, individual mental health, relapse prevention, harm reduction, and substance use counseling based on motivational interviewing and cognitive behavioral responses to mental illness and substance use
5.    Provide 24-hour crisis intervention on-call services on a rotating basis
6.    Provide 80 percent of treatment contacts in the community
7.    Document client progress to maintain a permanent record of client activity according to established methods and procedures.
8.    Participate in daily morning team meetings and treatment planning review meetings.
9.    Follow the established ACT policies & procedures and agency policies and maintain appropriate professional standards.
10.    Use individual client data to inform clinical decision-making and identify areas for quality improvement; use data to track client progress over time. 
11.    Share client data with clients to help facilitate shared-decision making and keep clients informed of their progress 
12.    Other related duties.

REQUIRED KNOWLEDGE and SKILLS:
1.    Experience understanding family systems based on diversity of cultural and ethnic backgrounds
2.    Experience understanding various evidence-based models and theories about psycho-education, motivational interviewing, severe mental illness, and recovery.
3.    Extensive knowledge of Assertive Community Treatment, family community support services such as NAMI, and community resources
4.    Use of Motivational interviewing, recovery, and trauma-informed approaches when delivering services
5.    Proficient in computers
6.    Highly organized and self-motivated
7.    Familiarity with and ability to use an Electronic Health Record system.
8.    Demonstrated commitment to diversity, equity, inclusion, and cultural humility.

QUALIFICATIONS:
1.    The Family Specialist must possess and maintain an active LMHC, LMSW, or LCSW with a Master’s degree in the Human Services field. 
2.    The Family Specialist will have experience using culturally competent treatment approaches. 
3.    Basic computer knowledge is required.
4.    Must be reliable and able to work independently and understand the importance of maintaining confidentiality.  
5.    Must have and maintain a valid Driver’s License and a driving record that is satisfactory to our insurance carrier.
6.    Must be and maintain clearance through the NYS Justice Center through fingerprinting for a Criminal History Records search.
7.    Must be and maintain clearance by the DCJS New York State Sex Offender Registry.
8.    Must be and maintain clearance by the Justice Center for the Protection of People with Special Needs Staff Exclusion List (SEL) and the New York State Medicaid Exclusion List.
9.    Must be and maintain clearance by the NYS Office of Children and Family Services (OCFS) for instances of child abuse and/or neglect. 
10.    Must be legally eligible to work in the United States.
