Position Overview:
This position is part of a grant-funded program to train, deploy, and engage Community Health Workers in COVID-19 response and in preventing chronic disease to improve community health, well-being, and resiliency.
This position will do outreach on COVID-19 prevention and chronic disease prevention through events, health fairs, tabling, block walking, and 1 on 1 and group presentations. This position will partner with schools, faith-based organizations, workplaces, and other partners to reach communities with health disparities including Hispanic/Latinx communities, African American/Black communities and low-income communities.
This position will work to improve community health through providing diabetes education classes, diabetes prevention classes, and physical activity opportunities in community locations. This position will also provide referrals to health and social services such as food access, housing, health care services and use a screening tool with individuals to determine resource and referral needs.

Additional Information:
- When public health responds to an emergency you may be assigned activities that are outside your regular job description. This may involve responding at any time, including nights and weekends with possible deployment to locations other than the Austin Public Health.
- This position is eligible for Public Service Loan Forgiveness (PSF) Program. This program is a federal program that is intended to encourage individuals to work in public service by forgiving the balance of their federal student loans. To qualify, the individual must have made 120 qualifying payments while employed by a qualifying employer.

Travel:
If you are selected for this position and meet the Driver Safety Standards in the City of Austin Driver Safety Program, you may drive when necessary to multiple locations as part of your regular job duties. Otherwise, you are responsible for getting to and from these locations.
