We are currently recruiting a Board-Certified Physician with an active Connecticut medical license. This is a full-time position supporting local skilled nursing facilities (SNF) within the southern Connecticut region.
CLAIM, LLC focuses on risk reduction, accelerated recovery and lower rates of hospital readmissions for our patients. The hands-on care we provide makes a meaningful difference in the lives of patients while improving overall quality of care. Our medical team partners with nursing homes to tailor individualized onsite coverage solutions to provide facilities with the highest level of service.
In this role responsibilities include:
- Managing new admissions
- Taking care of approximately 15 – 20 patients per day including: managing new admissions, daily rounding and completing notes based on agreed schedule with facility partners and patient needs
- Ultimately responsible for all patient care, and assumes primary care for the patients in the facility
- Maintains legal and ethical responsibility for directing care of the patient
- Attending quality assurance and policy review meetings on a regular basis
- Overseeing and support ongoing continuing education and in-servicing of staff
- Serving as clinical leader who helps ensure that exceptional patient care is delivered by fostering positive collaboration among care teams through open communication
- Assume Assistant Medical Directorships and Medical Directorships with increase in compensation through stipends

Qualifications
- MD/DO degree from accredited medical school
- Board Certified in Internal or Physical Medicine
- Active Connecticut medical license and DEA license in good standing
- Proficiency with EMR
- Compassionate, Intelligent, Motivated, Team Player
