Case Worker Position
Homeless Action Sonoma, Inc., is actively seeking a motivated and compassionate Case Worker to provide comprehensive support to clients facing various challenges. This role involves assessing client needs, developing and implementing case plans, and coordinating services to empower individuals and promote positive outcomes. Responsibilities
Client Assessment
Conduct comprehensive assessments of clients to identify their needs, challenges, and strengths. Establish rapport and build trusting relationships to better understand and address individual circumstances.
Case Planning and Management
Develop individualized case plans in collaboration with clients, outlining specific goals, action steps, and timelines. Regularly review and update case plans to reflect progress and address evolving needs.
Resource Referral and Coordination
Connect clients with relevant community resources, including housing assistance, healthcare services, employment support, and educational opportunities. Coordinate with external agencies and service providers to ensure a holistic approach to client support.
Advocacy
Advocate on behalf of clients to access necessary services and navigate bureaucratic processes. Collaborate with external partners to address systemic barriers affecting clients.
Crisis Intervention
Provide immediate support during crises, offering emotional assistance and connecting clients with emergency services when needed. Develop crisis management plans as part of the overall case strategy.
Documentation and Reporting
Maintain accurate and up-to-date case records, documenting client interactions, progress, and outcomes. Prepare regular reports for supervisors and funders to demonstrate the impact of the organization's services.
Collaboration with Team
Work collaboratively with other caseworkers, program managers, and support staff to ensure a coordinated and effective team approach. Participate in case conferences and team meetings.
Qualifications
- Previous experience in case management or social
- Strong interpersonal and communication skills
- Knowledge of local community resources and services in Sonoma County
- Ability to work with diverse populations and demonstrate cultural competence - Commitment to maintaining confidentiality and ethical standards
Job Type: Full-time
Pay: $20.00 - $23.00 per hour
