Description
The Driver is responsible for the transport of Clarity Care consumers to and from various agency locations, doctor appointments, work sites, Day Service programming, and other planned activities. They will also assist with grocery pickup and delivery for our Residential group homes.
ESSENTIAL FUNCTIONS AND DESIRED OUTCOMES:
- Performs duties related to transportation of consumers to and from various locations.
- Assists with grocery ordering, pick up, and delivery for our Residential group homes.
- Performs duties relating to the support, care, welfare, supervision and training of persons with intellectual disabilities and/or acute, chronic or long-term conditions.
- Follows safety and regulatory procedures related to transports and care of the consumers.
- Follows all documentation and reporting policies and procedures for any safety issues or significant events.
- Coaches, monitors, reports and documents consumer’s verbal and physical behaviors to ensure the safety and health of consumers and others.
- Maintains a safe and sanitary transport environment for all consumers.
- Responds promptly and competently in emergency response situations.
- Assists, escorts, and provides physical and other types of assistance needed by consumers during social and recreational activities.
- Provides opportunities sensitive to the needs, preferences, and choices of the consumer while promoting community recreational participation.

EDUCATION/QUALIFICATIONS:
- Must possess a high school diploma/equivalent and/or related work/life experience.
- Must be at least 18 years of age.
- Experience driving extended vehicles and/or wheelchair accessible vans is preferred.
- Must possess a valid WI driver’s license, and ability to meet agency driving standards. Must provide proof of auto insurance.
- Must pass a periodic WI Caregiver Background Check including Department of Justice Criminal Record Checks, and/or that of previous states of residence.
