Position Summary
Assists the Health and Wellness Director in various QMAP functions including identifying and reporting care needs, MAR audits, Medication department compliance, staff scheduling, and the overall supervision and development in QMAP care staff.
Essential Duties
- Communicates updates/change of conditions regarding residents to family members, case workers and doctors as appropriate
- Monitors the safety and well-being of residents. Reports any changes to Health and Wellness Director
- Oversees QMAP practice- audits, coaches, trains in order to ensure high level of compliance and accuracy.
- Ensures QMAPs observe residents for medication effects, side effects and medication indications.
- Oversees QMAPs to ensure medication carts, drawers and/or storage areas with miscellaneous supplies are organized
- Ensures all medications are available and ordered timely
- Assist HWD with quarterly MAR, physician order sheet and chart audits
- Audits and checks rooms for medications residents are self-administering and ensures compliance to guidelines
- Assist HWD with developing and updating care plans
- Assist HWD with evaluating and supervising resident care staff
- Assist with timely destruction of medications
- Assist the HWD with overseeing medication management to residents
- Maintains open communication with supervisor and/or peers to promote awareness of resident issues.
- Available for on call rotation
- Other duties as assigned.
Basic Qualifications & Experience
- Per the state of operation requirements, Medication certification; QMAP-A/L, Med Tech, Med Aide, Nurse Delegation
- At least one year of medication administration experience where exceptional skills were demonstrated
- Must be able to read, write and speak the English language.
- High school or Equivalent (GED)
- One (1) year experience in some level of elder care preferred
- Proficient computer skills preferred and knowledgeable in electronic health record management system.
- Strong self-management and organization skills
- Strong multi-tasking abilities with the ability to handle competing deadlines, flexible and adaptable.
- Ability to effectively handle difficult and sensitive issues and maintain confidentiality
Working Conditions / Physical Requirements
- Moderate physical activity required by handling objects up to 50 pounds occasionally and/or up to 20 pounds frequently.
- Constant standing, walking, and grasping. Frequent stooping and reaching above shoulder heights, occasional sitting, crawling, kneeling, and squatting.
- May have exposure to blood borne pathogens, body excretions and communicable diseases possible.
- Possible exposure to household chemicals.
- Sits, stands, bends and moves intermittently during working hours.
- May be subject to frequent interruptions.
- Interacts with clients, family members and visitors frequently.
