Position Summary:
The Care Coordinator, level one is responsible for facilitating the completion of the patient care coordination in multiple areas supporting Providers, Nursing, Medical Assistants, front desk, and handling inbound calls for health center(s). The referral/call center Director will direct assign and manage this position in alignment with operations and medical. The emphasis of this role is to expedite patient flow and related processes. Actively supports and participates in quality improvement to ensure efficiency, accuracy and facilitate patience access to care. This position provides outstanding service while meeting or exceeding departments performance standards.
Essential Functions:
- Promptly answers in-coming calls, greets caller, identifies self and the agency, and helps. Provides quality customer service on every call. Probes for proper calling routing and completes call routing to ensure customer reaches intended destination.
- Serves as operator for Clinica Sierra Vista inbound and outgoing calls and ensures the patient is transferred or referred to the correct department.
- Responsible for appointment scheduling / rescheduling. Answers phone in appropriate manner. Probes for all required patient/customer information, including demographic and appointment needs. Pre-screens for ability to meet financial obligations of visit checks for eligibility to appropriate special funded programs.
- Responsible for pre-registration of patients to include entry of all demographic information. Communicates financial requirements to patient/customer such as co-pay so that patient/customer is prepared. Verify insurance eligibility as allowed.
- Responsible for customer/patient follow-up to include upcoming appointments, re-scheduled appointments and resolution of patient/customer inquiry.
- Participates in Quality Improvement activities and training programs as required and/or appropriate. Maintains confidentiality of all patient information received verbally, electronically or in writing.
- Must be willing and able to change work shifts to meet patient and department needs.
- Monitors, distributes, and handles incoming and outgoing faxes
- Correspondence (Records, No Shows, daily inter-office mail)
- Enters Behavioral Health Screening scores into I2I
- Ensuring specialty reports are tasked to appropriate provider for review and close the referral in I2I
- Updating of the medication list as appropriate
- Scanning documents as directed.
- Contact/expedite connection with language line as directed
- Other related duties as assigned
Other Responsibilities:
- Assist patients with mobility issues
- May be asked to work at any CSV clinic or location as directed
- Escort patients to vitals area and/or exam room, lab, x-ray, etc.
- Other duties as assigned
Qualifications:

Full vaccination for NOVEL CORONAVIRUS (COVID-19) is required.
- High school diploma or completion of General Equivalency Degree (GED)
- Must be computer proficient and able to appropriately utilize an EHR/EPM system
- Able to type a minimum of 25 WPM and 10-Key
- Excellent customer service skills
- Good communication skills; ability to speak clearly, concisely and respectfully over the telephone.
- Precise and conscientious about details.
- Ability to handle multiple tasks well and to work well under pressure.
- Ability to take direction and work well independently and with others.
- Must be flexible and have ability to adapt and anticipate clinic needs
- Willingness to take on the responsibilities to assist the medical team in completing forms.
- Exceptional decision-making skills
- Medical Assistant certification preferred, but not required.
- CA driver’s license and clean driving record.
