A reputable acute care hospital in LosAngeles, California is actively hiring for a Case Management Nurse Manager to lead their Case Management team of 35FTEs. The Case Management Department is a comprehensive team of Care Managers, Discharge Planners, Care Coordinators and RN Utilization Management. The RN Manager, Case Management will work at a 150 bed medical center known for its high Leapfrog scores and this hospital recently began their Journey to Magnet.
The Hospital Case Management Manager will work alongside Case Management Directors and Care Continuum Leaders to design and implement best practices related to case management. The Case Management Nurse Manager will collaborate with medical directors and senior hospital leaders to assess local resources and appropriate discharge planning. Moreover, the Case Management Clinical RN Manager will improve hospital metrics such as Average Length of Stay (LOS), Total Hospital Days, and Readmission Rates. Effective usage of high-risk patient identification programs and readmission reduction programs will ensure the RN Manager, Case Management is effective in this crucial role.
