CSO is looking for a Hospital Services Supervisor to provide behavioral health services including consultation, support, assessment, intervention and discharge planning to individuals and families at Cooley Dickinson Hospital (CDH).

Community Behavioral Health Centers (CBHCs) are responsible for providing a full range of client-centered and family-centered services and interventions for mental health, substance use disorders, and co-occurring disorders. CBHCs serve as an entry point for timely, flexible, person-centered, high-quality mental health and addiction treatment on an urgent and ongoing basis.

Our Emergency Service Programs (ESPs) offer clients behavioral health assessments, intervention and stabilization (for youth, families and adults,) 24-hour availability, and are a preferred alternative to hospital emergency departments. We offer these services to all individuals regardless of insurance or ability to pay. We provide daily assessment and brief counseling, case management, nursing support and peer support. We have recovery and wellness focused groups, psychiatric consultation when needed and also provide discharge planning.

Responsibilities

1. Provide psychiatric and/or drug/alcohol evaluations, consultations and referral services for CDH patients including formulating a DSM IV diagnosis in medical units.

2. Assist patients/families through coordination of referrals and assist in resource linkage to maximize patients' abilities to be treated in the least restrictive setting.

3. Provide any documentation required by CDH to client medical record.

4. Coordinate in-hospital activities with Director of Behavioral Health at CDH.

5. Provide information and consultation to physicians and nursing staff as requested on behavioral health related concerns for medical patients. Facilitate hospital transfers and discharges when appropriate (behavioral health issues present) for medical units during off-hours.

6. Seek external information to supplement clinical assessments. This includes contacting providers, family, friends and significant supports of the individual being evaluated.

7. Request expert consultations including supervisors, psychiatrists, CDH staff and CSO staff that are available for specialized consultation. Examples include consults to assess acuity, medication needs, resolve ethical conflicts, cultural questions and risk management.

8. For the Medicaid-under-21 population, provide Mobile Crisis Intervention using, wrap-around principles, and using established MCI procedures. As hospital-based clinicians, HS Supervisors will intervene primarily in the Hospital but may provide follow-up care in the community.

9. Facilitate admissions to CDH inpatient mental health (W-5) following established procedures.

10. Consult with a CSO Supervisor or Director on:

  • All CCS admissions.
  • Discharges to the community when there is a risk issue or a section 12 involved.
  • All clients under age 21.

11. Consult with a Psychiatrist (either CSO psychiatrist or psychiatrist treating client) on:

  • Patients evaluated under a section 12 who will be discharged to the community.
  • Patients for whom medical staff have requested a psychiatric consult.
  • Other concerns as needed.

12. Comply with JCAHO, DMH, DPH and other licensing authorities, hospital, and third-party payor regulations regarding record keeping, reimbursement and pre-authorization requirements.

13. Complete all initial and ongoing credentialing requirements for those hospitals where employee will be providing services including obtaining required vaccinations, tox screens, and safety/health education competencies. (Agency to facilitate titres and screens; any necessary vaccinations/x-rays to be employee's responsibility.)

14. Maintains established CDH hospital policies and procedures, objectives, confidentiality, quality improvement program, safety, infection control, compliance and environmental standards

15. May occasionally be assigned shift duties at the CSO community-based site when needed as long as it does not deter from CDH shift. Also, when available and if necessary, may conduct evaluations in the ED.

16. Perform Site-based supervisory and clinical duties when assigned. Site-based functions will be conducted as described in the Crisis Supervisor job description.

Qualifications

Clinical Master's degree with independent license (LICSW or LMHC) required, with five years' or more experience treating individuals with mental illness in outpatient, residential, crisis intervention, or psychiatric inpatient setting post-masters degree experience preferred. In some cases, an LCSW or significant experience may be acceptable while working towards independent licensure. Ability to work with computers using Microsoft software, Adobe Acrobat, and web-based programs including basic data entry skills. Must be experienced in assessing individuals and managing appropriate dispositions. Must be able to triage and direct workflow, be familiar with community resources and possess strong organizational, interpersonal, communication, computer and phone counseling skills.
