Job Title: SSI, Access, and Recovery Case Manager
Classification: Non-Exempt
Salary Range: $40,000-45,500
Reports to: Project Director
SUMMARY:
The SOAR case manager is responsible for aiding clients in applying for social security benefits, referring, and linking clients to other resources clients have identified as areas of need, as well as community outreach to the target population.
DUTIES AND RESPONSIBILITIES:
- Provide comprehensive SSI/SSDI application assistance for eligible individuals who are experiencing or at risk of homelessness.
- Conduct outreach and other engagement strategies to unsheltered and sheltered populations to increase access to, and participation and retention in, harm reduction, case management, treatment, and recovery support services.
- Work closely with community collaborators, including private and public agencies to identify eligible clients and to arrange services.
- Develop a comprehensive individual service plan, with the client, outlining potential barriers that they may face in successful attainment of services in their recovery process.
- Monitor client’s adherence to substance misuse services.
- Complete all required documentation; maintain appropriate information in client files.
- Track success of clients to ensure the required outcomes of the grant are met.
- Educate community on services and program availability.
- Provide navigation support services and case management to ensure linkage and follow-up for recovery housing, childcare, vocational, education, and transportation services.
Requirements
EDUCATIONAL & EXPERIENCE REQUIREMENTS:
- Associate degree, or higher.
- At least five years’ experience with people with substance abuse or mental health histories and at least a year of experience with the identified population.
- Experience in online reporting a plus. Bilingual preferred.
SKILLS & ABILITIES:
- Strong interpersonal and communication skills and the ability to work effectively with the target population in diverse settings.
- Knowledge of community agencies and resources.
- Good communication skills, such as listening well, and using language appropriately.
- Indigenous to the local community, familiar with drug use subculture and other target populations, and ability to project a positive image and trusted as a source of information in the community.
WORKING CONDITIONS:
- Available to work flexible schedule which may include some nights and/or weekends.
- Work will be performed primarily in offices, encampments, homeless shelters, and automobiles that will vary considerably about the extent of heating, cooling, or physical comforts/protections.
- Amount of travel required is up to 50% of time.
- Able to lift a minimum of 25lbs.
