Coleman Health Services, a nationally recognized not-for-profit provider of Behavioral Health and rehabilitation programs is seeking a bachelors level QMHS, Case Manager to provide client and community support. Highly skilled Case Manager working alongside and collaborating with the I Promise HealthQuarters, which include LeBron James Family Foundation, Peg's Foundation, Akron Public School and AxessPointe.
Duties include:
- Provide case management and wraparound services to Summit County youth, adults and families.
- Provide community-based care in a variety of settings, including Summit County APS students and LeBron James Family Foundation-supported facilities.
- Have strong clinical documentation skills and be able navigate documentation within an electronic health record.
- Work together with a team of counselors/social workers, case managers, peer support specialist and the Director to provide prevention, sustainability, and reduce trauma in youth and their family members.
- Be a member of the newest and most valued service line for youth as we increase awareness and bring hope to Summit County Youth and their families.
- Be a member of a program striving for National Recognition, with heavy emphasis on cultural humility and trauma-based care.

Qualifications:
- Bachelor's degree in a social service-related field OR 3 years' experience in a similar position
- Willing to get NPI and Medicaid Billing number (we'll help!)
- Maintain excellent driving record
