The Colorado Coalition for the Homeless seeks a Behavioral Health Clinician to provide care to individuals with disabilities in the designated single “program units” of the CCH housing facilities and/or in off-site units.  The goal is to assist these individuals in maintaining their housing and to provide the services, linkages, and clinical interventions necessary to prevent them from returning to homelessness and to live as independently as possible. This position will be responsible for the intake, assessment, treatment planning, and service provision to adult individuals with disabilities. This position is located at 25 E. 16th Ave., Denver, Colorado 80202 and reports to the Associate Director.  Qualified candidates are eligible for the National Health Service Corps (NHSC) Loan Repayment Program.  Licensed health care providers may earn funds towards student loans in exchange for a two-year commitment at an NHSC-approved site through this program.  NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date.  The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance.  Please note that a start date will be scheduled once credentialing and privileging is complete.   Coalition Benefits   In addition to the unique challenges and extraordinary rewards of our work, eligible Coalition employees enjoy a rich benefits package, including:  Health insurance coverage on the first day of employment: full-time employees contribute only 1% of their earnings for their own health coverage and no more than 4% of their earnings for coverage of eligible dependents. Works collaboratively as part of a multi-disciplinary team to serve clients in a comprehensive, effective, and caring way. Provides ongoing clinical case management services to homeless persons with psychiatric diagnoses, including severe and persistent mental illness, and who may have a co-occurring substance-related disorder. Conducts biopsychosocial assessments, intake screenings, makes appropriate diagnoses according to DSM-5 guidelines, and creates treatment goals with the client. Facilitates therapeutic meetings with clients in individual and group settings. Maintains accurate electronic health records with timely documentation and participates in audits and projects to help improve and maintain accurate records that meet standard of care. Records all services in a timely and complete fashion in the EHR. Maintains up-to-date records in EHR. Records telephone and other communications as needed in the EHR. Manages tasks and scanning as needed to have a complete and current chart. Maintains accurate information regarding client’s insurance status. Utilizes the EHR and any future upgrades as directed.  Monitors and maintains productivity required for position. Completes and maintains requirements to fulfill internal credentialing and privileging, which is necessary to provide clinical services. Meets formally with Program Manager regularly for ongoing supervision. Attends and participates in clinical and administrative meetings as deemed appropriate and necessary for the job. Assists others with work that needs to be done in moments of urgency or crisis. Completes Medicaid authorizations and client database entries as well as any other reporting requirements necessary to obtain and sustain funding and provide services to individuals served in the program. Assists clients in identifying and accessing goods and services for both immediate and long-term needs, such as physical health needs, psychiatric medication, etc., and facilitates connections with other service providers to help client meet needs.  Provides needed transportation for clients as appropriate. Navigates the Social Security, benefits, Medicaid, and Medicare systems on behalf of clients and serves as a liaison with collaborating agencies. Assist residents in problem-solving issues which may threaten their housing, and when appropriate serves as liaison between client and property management staff to advocate for client’s housing stability. Covers for other Behavioral Health Clinicians or Case Managers as necessary at the same or other CCH locations. Maintains current and complete COVID‐19 vaccination(s), as defined by the Centers for Disease Control and Prevention and local health authorities. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. ​Qualifications Summary   Master’s degree in social work, counseling, or similar field. Licensed as an LCSW or LPC preferred. If not yet licensed, on track to do so as an LPCC or LSW.   Experience and interest working with disenfranchised persons.  WHERE A CANDIDATE IS PLACED IN THE COMPENSATION RANGE DEPENDS ON TOTAL RELEVANT YEARS OF EXPERIENCE
