To excel in the role of a Registered Nurse at Comfort Home Care in Leominster, you must possess a diverse skill set. You'll need to leverage your expertise in conducting initial and ongoing assessments to identify clients' medical needs and functional levels effectively. Your ability to initiate and adjust care plans, conduct OASIS assessments when necessary, and implement appropriate nursing interventions will be crucial. Coordination skills are key as you communicate changes in clients' conditions to physicians and collaborate with healthcare providers and community resources.

Knowledge and skills required for the position are:

• RN provides initial and ongoing assessments to identify the client's medical needslevel of functioning and ability to communicate and learn.
• RN initiates the plan of carenecessary revisionsand performs OASIS assessments at appropriate time points when applicable.
• RN Initiates and performs appropriate nursing interventions.
• RN coordinates servicesinforms the physician and other personnel of changes in the client's condition and needs.
• RN collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge.
• RN Prepares clinical and progress notes and submits all required paperwork in a timely manner according to agency policy and procedure.
