CHI Health would love to add a Clinic RN (Nurse) to our Internal Medicine team located at our CUMC-Bergan campus off of 77th & Mercy handling all the Wellness Nursing duties for our Medicare patients!
Hours: Monday-Friday between 7a-5p, no weekends, no holidays & no call!

Expectations:
- This position is for a Annual Wellness Visit RN. Wellness RN Work as part of a team to provide Annual Wellness visits to current Medicare patients past their 1st year of eligibility.
- Help the patient and provider to create custom 5-10 year preventative screening plans for Medicare patients.
- Provide education to help the patients understand how preventative care impacts their health.
- Performs general nursing care to patients by following established clinic workflows.
- May perform specific nursing care as it relates to specialty of the practice.
- Educate patients and families about health status, health maintenance, and management of acute and chronic conditions.
- Document patient assessment and intervention data using established medical record forms/automated systems and documentation practices.
- Performs other duties within scope of role as assigned
Qualifications
Minimum Qualifications:
- Current Registered Nurse (RN) license that allows you to practice in our state
- Graduate of an accredited nursing program
- Basic Life Support (BLS) from the American Heart Association required
