Community Bridges is looking for a full-time Behavioral Health Case Manager!
We are a non-profit agency founded in 1982 with a mission to advance the integration, growth, and interdependence of people with developmental disabilities within their own communities. Building on the strengths of over 1,100 individuals in the greater Capital Area and their families, we work to identify barriers of access to find creative solutions to develop meaningful community engagement.
The Behavioral Health Case Manager assists individuals and families by helping them to develop and maintain positive connections with Community Bridges. The Behavioral Health Case Manager uses a person-center approach to assists individuals and families to identify, plan for, and follow through with needed supports and services to live the life that want in the community they choose. The Behavioral Health Case Manager provides quality assurance monitoring and oversight to ensure compliance, service satisfaction, and that people are treated with dignity and respect.
Qualifications:
- Education: Associate's Degree preferred.

Experience: Two years' experience in human services, developmental services, special education, or a related field required.
Compensation and Benefits
There is a $1000 sign-on bonus for this position - $500 will be paid at the time of hire and $500 will be paid after 6 months in the role. *Please note, sign-on bonuses are for new hires only. Re-hires and transfers are not eligible at this time.
Paid Time Off: For a full-time employee working 40 hours a week, 264.16 hours of Paid Time Off is received per year! PTO days will increase over the years based on longevity and there is no waiting period to start using your PTO!

Specific Duties and Responsibilities:
- Maintain a caseload of at least 75% individuals who experience intensive behavioral challenges.
- Develop and maintain knowledge of current resources in NH that are available to families with individuals who exhibit behavioral challenges.
- Provide consultation to IFS case management staff as requested by immediate supervisor or Director of Case Management.
- Engage in regular consultation with clinical team members in addition to regular supervision by direct IFS supervisor.
- Attend Clinical team meetings and provide regular updates on individuals who are listed for regular review by the IFS clinical team.
- Provide multi-dimensional support to people who are receiving services under the Home and Community Based Services waiver.
- Maintain current and accurate knowledge of how to access and effectively use local, state, and federal resources and benefits; ensure continuous eligibility for needed benefits; and collaborate with other community based resources to create a holistic continuum of care and support.
- Assist individuals and families to obtain funding for needed services.
- Engage individuals and families in the process of person-centered thinking and planning to discover personal values, preferences, dislikes, strengths, needs, hopes, and dreams; identify current support needs and opportunities for independence; plan for future needs; identify and encourage natural supports; create goals; and problem solve. Encourage self-advocacy and empower individuals and families to live the life they want.
- Schedule, plan, and facilitate a variety of meetings frequently.
- Maintain, at minimum, monthly contact with individuals and/or service providers.
- Perform administrative duties and provide quality assurance oversight for all support services by completing site visits; facilitating quarterly service reviews; evaluating individual, family, and guardian satisfaction; and reviewing documentation.
- Maintain an up-to-date record of communications, required personal and health information, historical information, demographics, and contact information within the required electronic system.
- Ensure HIPAA compliance and maintain a person-centered approach. Consistently present self in manner that demonstrates respect and consideration for all people's rights, preferences, and choices.
- Report concerns of abuse, neglect, and/or exploitation to the appropriate entities.
- Make referrals and recommend necessary services or supports. Maintain knowledge of waitlist status.
- Attend trainings and educational opportunities. Educate others of knowledge when appropriate. Provide education to help foster an integrated and accepting community.
- Assist in the orientation and training of new staff. Share resources and knowledge with peers.
- Perform other duties and tasks as assigned.

Work Environment and Physical Demands:
- Work Environment- Occasionally hectic with occasional periods of high stress. Occasional to moderate short deadlines and coverage require flexibility in schedule.
- Physical Demands- Must be able to sit or stand at a workstation.
Travel:
Ability to travel and have access to personal vehicle during all scheduled work hours. Must possess and maintain a NH license and an acceptable driver's record in accordance with Community Bridges standards.
