JOB DESCRIPTION

JOB TITLE: Community & Family Support Coordinator

POSITION SUMMARY:
A direct service position responsible for providing support, life skills and/or parenting skills training to independent adults with developmental disabilities. Services are in home and in the community and require outreach services with community and generic resources. Typically reports to Program Managers. Work hours are flexible and can vary, may include holidays, weekends and evenings depending on consumer needs/goals.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Provide ongoing support and training to consumers; establish and maintain a constructive and appropriate relationship with consumers; assess and take appropriate action to ensure the health and safety of consumers; assist in the utilization of available community/generic resources and support services to best achieve consumers/agency goals and objectives; complete daily record keeping; participate in the development and implementation of service reports, individual support plans and assessments.

QUALIFICATIONS:
Ability to interact well with others; write legibly; read and follow directions; work independently; perform each duty satisfactorily and under pressure; minimize risks of endangerment to the health, safety, and well being of consumers; perform first aid and cardiopulmonary resuscitation; possess a valid CA driver’s license, automobile insurance and good driving record.

EDUCATION/EXPERIENCE:
Minimum: High school diploma/GED and 1 year of human services employment/volunteering or equivalent experience.

Job Type: Part-time

Pay: $15.00 - $16.00 per hour

Education:

• High school or equivalent (Required)

Experience:

• Experience with children: 1 year (Preferred)
• Child welfare: 1 year (Preferred)
• Social work: 1 year (Preferred)

License/Certification:

• Driver's License (Required)
