In order to continue to reach our goals as a company, we are seeking an experienced MDS Coordinator to join our team. Candidates should be able to perform the essential responsibilities as well as meet the qualifications listed.
Essential Duties and Responsibilities:
· Complete the RAI process timely, for each resident in the facility.
· Conduct the Care plan meeting and formalize the written care plan utilizing the information obtained at the meeting. Include the resident and family.
· Participate in the ongoing quality assurance program for the nursing department.
· Participate in Nursing "on call" rotation.
· Must be able to lift/transfer 50 or more pounds
Qualifications:
· Must be licensed as an R.N. or L.P.N. in this state.
· Experience in skilled nursing facilities/nursing homes
