Location: Cape Girardeau, MO - Cape Girardeau County
Full Time: Non-Exempt
Work Hours: Monday-Friday: 8:00am - 4:30pm. Some evenings may be required as necessary dependent upon agency and client need.
Summary: As a care coordinator you would be responsible for helping with referrals to the Integrated Treatment of Co-Occurring Disorders (ITCD) program. You would also provide case management and in-home community based coordination of service for co-occurring clients, serve as a point of contact for referrals to ITCD, assist with getting referrals enrolled into CPRC and ITCD, and continue with case management services until the client can be assigned to a CPRC Case Manager.

Minimum Qualifications:
- Bachelor's Degree in a Human Services field (see below),or;
- Social Work, Psychology, Nursing, Education, Criminal Justice, Recreational Therapy, Human Development and Family Studies, Counseling, Child Development, Gerontology, Sociology, Human Services, Behavioral Science, and Rehabilitation Counseling.
- An individual with any four-year combination of higher education and qualifying experience, or;
- An individual with four years of qualifying experience,
Qualifying experience must include delivery of service to individuals with mental illness, substance abuse disorders, or developmental disabilities.
- Individuals must have reliable transportation, continuous valid driver's license, and proof of continuous auto insurance
- Travel in own vehicle is required.
What your duties look like:
- Coordinating Services for Co-Occurring Clients: You will be coordinating community support services to clients enrolled in the ITCD program. This includes maintaining referrals, ensuring completion of assessments, participate in inter-agency treatment planning, and assisting the client in accessing services or resources.
- Documentation: You will utilize collaborative documentation to show the services that you have provided to your clients. This also includes your initial checklists, quarterly evaluations, and annual evaluations. It is important to note that if you meet with a client, the documentation is due by the end of the following business day.
- Team Based Approach: Having a positive attitude and being a team player is extremely important in this position. You are a member of your client’s treatment team, and usually the first call when questions arise regarding your client. Working with wide variety of people means that being a people person is important in this position.

- The necessity of mental health services, and the impact that it can have on the community. You follow a person-centered approach and understand that meeting the client where they are is an important aspect to treatment.
- The importance of Organization, Time Management, and Detail Orientation. This job requires you to manage your time wisely, and without organization it will be difficult. Since you are in control of your schedule, it is important to stay on task and have the independence to ensure that you are able to meet the needs of the job.
- How to be a team player. As mentioned above, as a care coordinator, you are part of a client’s treatment team. Being a treatment team member requires a team-based approach and the ability to work well with others in the organization.
- How to (effectively) use a computer. This position uses an Electronic Health Record for documentation, and if you’re not meeting with a client, you are utilizing your laptop.
What you will learn (if you don’t know already):
- How to provide case management services to clients with a mental or behavioral health disorder and a substance abuse disorder.
- How to utilize your time management skills for collaborative documentation to ensure all notes are being completed on time; as well as how to manage your client load and to ensure you are providing quality, and timely services.
- How to be an Electronic Health Record (EHR) superstar – we make sure that you are comfortable and receive more than enough training in order to have you feel comfortable using our EHR system.
- The importance of self-care, not only to teach to our clients, but also for yourself.
How Success is Measured:
- Direct Service: Consistency in meeting with your clients to provide services and to ensure their needs are met (typically ~ 25 hours/week).
- Documentation: Providing quality, thorough, and concise documentation. Ensuring that your documentation is completed within the designated timeframe and that your Initial/Quarterly/Annual checklists are completed.
- Communication: Communicating effectively with your clients, co-workers, supervisor, and treatment team members.
- Time Management: Ensuring that you are not only being on time for all scheduled meetings/supervisions, but also to client appointments and managing your time in reference to your documentation.
About the team:
This role would report to the Co-Occurring Program Manager and would also work closely with other members of the Co-Occurring Treatment team, and the other members associated with the clients Treatment Team.
