Description

Callen-Lorde Community Health Center seeks a dedicated, flexible individual to join our staff as the Supportive Case Manager of Care Coordination services. The Supportive Case Manager is responsible for supporting, assisting, and ensuring that clients’ biopsychosocial continuum of care needs are identified, and services are appropriate and monitored.  The Case Manager will facilitate client’s transition into primary medical care, and work closely with a medical and mental health care team, and other community based organizations to ensure appropriate linkage to supportive services as well as retention in care. Callen-Lorde is open Monday through Saturday.  Weekly evening and occasional weekend hours will be required

Qualifications

  • College degree (BA/BS) in health/social services or related field required.
  • Experience in social services, case management and/or entitlements work.
  • Familiarity with LGBTQIA, HIV and health-related issues and community resources.
  • Strong computer competency, including knowledge of Electronic Medical Records, Computerized Appointment/Billing Programs and other Microsoft Office Programs.
  • Strong customer service orientation.
