Responsibilities:
We are looking for a passionate Diabetes Care and Education Specialist to join our growing team! In your role, you will perform essential care for our patients - helping them to understand the comprehensive management of blood glucose in patients with diabetes. You will also assist with the continuum of care, keeping the patient's primary physician informed about the patient's status and providing ongoing instruction to the patient as needed.

Qualifications:
Education and Experience Minimum Requirements

High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required
If qualifying as a Registered Nurse (RN), graduate of approved nursing program required

If qualifying as a Registered Dietitian (RD):
Bachelor's Degree in Dietetics, Food and Nutrition, or related field granted by a U.S. regionally accredited college or university required
Completion of a didactic program in dietetics and supervised practice program approved by the Commission of Dietetic Registration (CDR), the credentialing agency for the Academy of Nutrition and Dietetics required
Experience in direct patient care with diabetes patients required

Licenses and Certifications

CDCES - Certified Diabetes Care and Education Specialist required within 2 years of hire or transfer into position
BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
One of the following is required
RN - Current State of California Registered Nurse license
RD - CDR Registered Dietitian
