Community Outreach and Marketer of Home Health

Community Home Care Services is growing, and we are looking for the perfect individual to fill our Home Health Community Outreach Marketer position. We need an outgoing, motivated, and professional individual to act as a liaison between our home health care agencies and other healthcare professionals within Fulton, Marshall, Pulaski, and Starke Counties. Requirements include:

• Applicants must be professional, outgoing, and comfortable with meeting new people every day
• Must have reliable transportation
• Must be extremely self-motivated and highly driven
• Must have excellent time management skills
• Experience with home care and/or marketing is preferred but not required, will train if needed

Responsibilities include:

• Identification of new patient referral sources
• Development and maintenance of existing and prospective professional relationships/accounts
• Identification of community outreach opportunities to promote our company as an active contributor and supporter of our area and its citizens
• Participation in care coordination to ensure the fluid transition of patients to and from other healthcare providers
• Regular communication with your office staff and clinicians to ensure continuity of care
