30-hour per week full time position - day shift
Summary: Responsible for implementing, coordinating, facilitating, documenting and evaluating the bereavement program and the delivery of bereavement services, which may include support group facilitation, presentations, events and providing grief support as needed.
RESPONSIBILITIES:
1. Implements the bereavement plan of care for survivors of hospice patients. Coordinates, facilitates, and documents the plan of care for survivors of hospice patients, which includes delivery of the bereavement support services through personal contact, calls, and letters.
2. Conducts routine and high-risk screenings for adults and children/teens. Provides, coordinates, facilitates, documents, evaluates, as needed: individual adult, child/teen and family grief support, anticipatory grief support, home/school visits, facility visits; adult and child/teen support groups.
3. Ensures adequate facilitation of support groups. Provides or facilitates on-going training, supervision, evaluation and support for the adult, child/teen grief support group facilitators and co-facilitators. Facilitates grief support groups as needed.
4. Acts as the bereavement department's representative to the interdisciplinary team meeting and serves in collaboration with the department head as liaison between other departments at Community Hospice. Attends weekly IDT meeting to receive survivor risk assessment information from team.
5. Develops, coordinates, and/or facilitates community grief/bereavement education. Implements multiple grief education programs, staff/community memorials and other bereavement programs such as the “Coping with the Holidays” workshop.
6. Partners with the Volunteer Specialist/Director of Bereavement Services in the training, supervision, evaluation, discipline and/or termination of Bereavement Department volunteers as required. Assists the Volunteer Specialist/Director of Bereavement Services in evaluating hospice volunteer’s performance to assure that it meets minimal Medicare standards.
7. Assists with the Bereavement Department planning, documents service delivery utilization, and outcome information. All documentation is to be completed in the specified timeframe.
8. Assists with grant development and/or other forms of reimbursement for grief/bereavement services. Forms and submissions are to be completed in the specified timeframe.
9. Collects data sufficient to meet both internal and external organizational needs and prepares monthly bereavement statistics report for internal and external stakeholders. Monthly Statistics Report due on the first business day of the following month for the previous month.
10. When requested by the Volunteer Department or Public Relations and Marketing Department, provides information to individuals and community groups about hospice services and opportunities. Serves as a community resource through presentations to groups and individuals.
11. Follows all Community Hospice policies and procedures including, but not limited to, safety, infection control, privacy and confidentiality.
12. Performs other duties as assigned, including work in other areas to cover absences or relief to equalize peak work periods or otherwise meet the needs of the organization.
Qualifications
· Education consisting of high school diploma or equivalent, currently possessing a counseling or certified bereavement training certificate by a recognized state or national organization.
· Extensive knowledge of grief and bereavement processes is required.
· Minimum one-year experience in a hospice or social services setting is preferred.
· Minimum one year in a position that required coordination and/or administration of services.
· Excellent computer data entry and database maintenance skills required.
· Valid California driver’s license, reliable transportation, and current auto insurance is required.
· Bilingual English/Spanish preferred.

Pay: $22.50 - $30.24 per hour
