Job Posting for Community Integration Specialist at Community LIFE
Position Duties: Assists the participant to maintain their independence in the community by assisting and encouraging the participant to plan, organize, coordinate and implementation of daily tasks in the home or community such as leisure activities/community integration/leisure outings/companionship. Work duties vary on a day-to-day basis and a flexible work schedule may be required. This position will enhance opportunities to increase a person’s abilities in their home and community. All participant activities must be approved by the Interdisciplinary Team and structured for the participants independence.
Requirements:
- A High School Diploma or GED
- Minimum of 1 year (12 months) of experience providing caregiving services for the frail or elderly
- Valid PA Driver’s license and reliable vehicle
