GENERAL DESCRIPTION OF DUTIES


The Lead Forensic Case Manager is responsible for the Management of the Transitional Case Manager (TCM) Program, the operations of the program, and the corresponding clients/customers. This individual oversees the TCM program, staff, and outcome measures and goals as well as provides specialized group and/or one on one counseling services to incarcerated individuals with substance use and or co-occurring disorders within the Clinical and Supportive Services.

ESSENTIAL JOB FUNCTIONS

  • Supervises staff, provide guidance, and workloads as assigned
  • Train new staff on process and documentation for services received and outcomes measures within the Clinical and Supportive Services.
  • Responsible for the oversight and delivery of services relative to the TCM program, outcomes and deliveries per contract with the Department of Mental Health and Addiction Services
  • Responsible for making autonomous decisions regarding care and services for clients/customers
  • Provides crisis intervention services in response to emotional and/or physical distress.
  • Provides diagnostic evaluations to customers in Prison using best practice assessment tools.
  • Ensures adherence to infection control practices.
  • Maintain client’s case load in Pre/Post incarceration.
  • Makes referrals to outside programs for medical intervention.
  • Provides counseling services to Substance Use individuals with co-occurring conditions and consults with licensed clinicians.
  • Assesses customers in order to formulate treatment needs.
  • Orients new clients to treatment program(s).
  • Conducts and or participates in treatment planning and individual and/or group counseling.
  • Identify potential customers and provide them with information about the Agency programs.
  • Develop, implement, and evaluate a comprehensive individual service plan (ISP) with each customer/client maintaining regular contact.
  • Provide weekly home or office visits to program participants as applicable.
  • Conduct urinalysis screening as required by program and/or funder.
  • Provide assistance and education in activities of daily living, support for employment/training search and job coaching.
  • Complete and maintain required paperwork and records on required systems STS advance MD, DDAP.
  • Maintain customer/client confidentiality.
  • Provide on-going liaison, referral and advocacy for customers.
  • As part of professional development attend all trainings and meetings as assigned.
  • Participate with community providers as it relates to programs contents area.
  • Provide or conduct presentations on programs to Incarcerated individuals based on Grant outcomes.
  • Maintains great relationship communication with DOC and DMHAS on incarcerated individuals
  • Completes referral progress reports and/or closing/discharge summaries
  • Maintains and disseminates listings of local AA, NA and CA meetings as needed

ADDITIONAL JOB FUNCTIONS


  • May require use of crisis intervention
  • Attend a minimum of credentialing hours of substance use related training per year.
  • Participation in planning and implementing a minimum of one monthly group activity for program participants, including but limited to: life skills, education, empowerment/support recovery groups.
  • Completes yearly mandatory training including confidentiality and HIPAA compliance.
  • Conducts presentations for/at assigned prison regarding program resources.
  • Must be available to work evenings, weekends, and other shifts as assigned.
  • Perform other duties as assigned.


MINIMUM TRAINING EXPERIENCE AND MEDICAL CERTIFICATIONS


Education:
Bachelor’s Degree in Social Work or related field from an accredited institution. A combination of college, certification, training and experience will be considered


Minimum Years of Experience:
At least three (3) years of experience with substance use.


Active Certification in good Standing:
Must have a valid driver’s license


Demonstrate skills:
Proficient in databases, MS Office, written reports and maintenance of case notes. Strong organization, motivational, time management and interpersonal skills. Specialized training with HIV/AIDS, Drug and Alcohol Abuse and/or counseling, Domestic Violence and/or any other applicable training related to field of work preferred.


Knowledge of:
Human Services, Criminal Justice, Court systems, substance use and target population(s) a must. Comprehensive knowledge of social services in Hartford with the demonstrated ability to work in collaboration with other service providers is required. Experience in facilitating groups and counseling background strongly preferred. Experience working with participants from diverse ethnic and economic backgrounds with the ability to interact and communicate effectively.

Yes, preferred Language: Spanish


Driving Required:
Yes, Agency Vehicle: No Employee’s Own Vehicle: Yes, with proof of auto liability insurance.

Active Driver License in good standing required upon hire and throughout employment
