Position Title: Care Coordinator – Residential Placement/Companion Specialist
Position Type: Full-time
Location: Statewide - CT
Employment Type: 12 Months/Year Round
Compensation: Hourly
Work Location: Hybrid/In-Person with state travel requirements
Qualifications: Experience with adults with intellectual and developmental disabilities/Autism; DDS residential services; or human services degree preferred.
Required Skills/Training/Certifications:
- A valid CT driver’s license with a good driving record; appropriate vehicle insurance.
- First Aid/CPR
- Proficient computer skills, including Microsoft Office

Essential Duties and Responsibilities:
1. Ensures all residential/companion services are aligned with State regulations and best practices. Serve as a liaison between the agency/individual/provider/family/care network etc.
2. Builds rapport and trust with providers, individuals served, and their families while conducting monthly home visits, care coordination; participate in team meetings, planning; support and promote linkages to community services to help resolve identified barriers through referral, advocacy and monitoring of services in accordance.
3. Maintain confidentiality of information regarding individuals served, providers, and families (in accordance with HIPAA and State & Federal regulations)
4. Participates in various collateral and community groups to promote agency programs and to network within the community
5. Assist individuals served identify support strategies that will enable them to adequately meet their goals and wishes
6. Provide support as needed to CCH providers, and individuals placed in their home.
7. Assure that plans address health and safety, housing and employment, social networking, scheduling and documentation of meetings and appointments, utilization of natural and community supports, and the quality of supports
8. Maintain up to date case records and submit all required documentation as defined by agency policies, procedures, and superviors.
9. Present ideas, suggestions, and needs to supervisor in a clear and objective way and adhere to all company policies
10. Accept management/supervisory directives and adjust promptly and calmly to new situations
11. Perform other duties as assigned
