We are searching for a Case Manager to join our team. This fulfilling opportunity lets you be a part of making a difference in the lives of many. Reporting directly to the Administrative or Clinical Supervisor, you will handle intakes, caseload management and discharges of clients. This involves managing client records, keeping abreast of client care and communicating closely with therapists, families, applicable CONCERN staff and other agencies involved with each individual.

This is an office, home and community based position with work days and hours based upon the needs of the client.

What Do I Need?
Education and Experience

Bachelor’s degree in Sociology, Social Work, Psychology, Gerontology, Anthropology, Political Science, History, Criminal Justice, Theology, Nursing, Counseling, Education, or a related field.
OR
Registered Nurse
OR
High School diploma or equivalency and 12 semester credit hours in Sociology, Social Welfare, Psychology, Gerontology or other social science and 2 years of experience in public or private Human Services with 1 year of mental health direct care experience.
OR
Consumer or family member who has 1 year of experience as an advocate or leader in a consumer or family group with a High School diploma or equivalency.

Other Requirements

  • You will maintain required state and federal criminal and child abuse clearances, credentials and/or licenses
  • Driver’s license and reliable transportation are required.
  • You will be familiar with community support systems, health care and human service resources.
  • You will be skilled at crisis intervention and plan development.
  • You will work as part of a team and independently taking initiative to create solutions when problems or concerns arise.
  • You will be proficient in Microsoft Office Suite or related software.
  • You will be familiar with or comfortable working within Electronic Health Record (EHR) systems.
  • You will demonstrate excellent organizational skills with a strong attention to detail.
  • You will have clear written and verbal communication skills

  • You will maintain confidentiality of all sensitive information.

What Will I Do?

  • You will deliver direct crisis intervention services and supports to consumers and families through phone and face-to-face contacts within an office, home, or community setting.
  • You will perform intakes, assessments, and evaluations for consumers. This includes assessing problems/needs and obtaining background information to refer consumers for appropriate treatment within the service delivery system.
  • You will create and implement crisis intervention management and safety plans as necessary to promote the health and safety of consumers, families, and others.
  • You will remain knowledgeable of the local Human Service system and will actively coordinate services with, and link consumers and families to, various services and supports available.
  • You will assist consumers and families in managing crisis, stress, conflict, and potential health and safety risks through the direct delivery of crisis intervention services and support.
  • You will conduct follow-up and ongoing support as determined clinically appropriate and needed until consumers and families are established with ongoing formal services and informal supports necessary for them to maintain stability and eliminate the need for crisis services.
  • You will coordinate medical and psychiatric consultation for consumers as determined clinically needed during the delivery of crisis intervention services.
  • You will be responsible for timely and accurate submission of consumer information and service delivery documentation following program standards and agency policies and procedures.
  • You will collaborate regularly with partnering providers, internal/external referral sources to coordinate services and maintain continuity of care.
  • You will demonstrate a commitment to diversity, equity, and inclusion through continuous development, modeling inclusive behaviors, and proactively managing bias ensuring a welcoming, safe environment for all employees, vendors, clients, and their families.
  • You will engage in regular supervision opportunities with Crisis Supervisor.

Hours of Work
This is an office, home and community based position with work days and hours based upon the needs of the client.
Typical work hours are Monday – Friday during business hours.
Ability to participate in the on-call crisis intervention schedule as needed. (preferred)
