HOW OUR STNAs CONTRIBUTE:
ConcertoCare Certified Nursing Assistants (CNAs) deliver in-home care to patients and provide virtual clinical care support under the general supervision of a registered nurse or licensed medical provider. This position reports to the practice manager.
WHAT OUR STNAs DO:
- Conduct in-home visits to provide direct patient care under the general supervision of a licensed clinician, including patient observation, collection of vital signs, and administration of health screenings.
- Conduct in-home visits to facilitate virtual visits with a remote licensed medical provider.
- Provide virtual support for a panel of patients, including pre-visit chart prep, tracking diagnostic and referral results, managing quality measure orders and results, coordinating routine care scheduling needs, and ensuring appropriate follow-up.
- Coordinate and support patient care by performing outbound calls and tasks as directed by a supervising clinical practitioner.
- Process and track orders and referrals placed by our providers as directed, ensuring patients complete orders and referrals per their treatment plan.
- Maintain up-to-date electronic health records for vaccination history, preventative care, and other quality measures.
- Deliver exceptional customer service to our patients, caregivers, and clinical partners, including proactive, timely, and comprehensive communication from a holistic patient-centered perspective.
- Perform additional clinical procedures, including specimen collection and diagnostic point-of-care testing, as permitted by State scope of practice regulations.[SQ1]
- Accurate and timely documentation of all patient care support activities in the medical record.
- Promotion of and participation in patient engagement and experience initiatives and efforts.
- Foster relationships with patients, caregivers, congregate living facility staff, and partnering organizations. Effectively coordinate patient care with caregivers and facility staff as needed.
- Active participation in team huddles and organizational meetings as requested.
- Adherence to compliance policies, procedures, and standards of conduct, including all applicable laws and regulations.
- Coverage for Care Navigator duties on an assigned basis, including front office administrative tasks. [GM2]
- Other market-specific duties as assigned.

QUALIFICATIONS
- Current certification as a STNA in the state of Ohio.
- At least one year of experience in the last 24 months as an STNA or Patient Care Technician in a hospital, home health/hospice, or skilled nursing/long term care facility.
- Current certification in Basic Life Support (BLS) required.
- Phlebotomy certification required.
- Ability to conduct patient home visits, with prior in-home experience preferred.
- Average to advanced computer and software skills required.
- Ability to travel within assigned area and access to reliable transportation, including to locations that may have limited access to public transportation; valid driver's license and proof of liability and property damage insurance on vehicle used is required.
Base Salary/ Wage Range $15.00 to $22.00 plus annual bonus. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown.
