Position Responsibilities:
- Embody the company standard as a high-quality care provider.
- Demonstrate strong leadership.
- Supervise, coordinate, and perform the activities of the Social Services Department.
- Assess, plan, implement and evaluate the social, emotional, and psychological needs of patients.
- Identifies and address psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families in a friendly and sympathetic manner.
- Collect data, conduct patient assessments and complete all state required documentation.
- Coordinate discharge planning.
- Coordinate the referral of patients and families to appropriate community, social service, other extended care and rehabilitation facilities and organizations.
- Maintain regular and on-going relationships with families and residents.
- Coordinate and direct care plan meetings.
- Communicate with physicians, nursing staff and other members of the team in the planning of patient care services, patient care plans and discharge plans.
- Communicate daily with Nursing and Admissions team regarding admissions, discharges and transfers. Utilizes problem-solving skills to assist residents and families in identifying potential barriers to discharge. Provides education, resources and training necessary prior to discharge from the facility.
- Educates patients and families on the general criteria for Medicare, Medicaid, Hospice benefits, and insurance coverage. Identifies potential financial barriers and collaborates with family and members of the leadership team to make recommendations for treatment and discharge planning with consideration to financial status, diagnosis, and discharge needs.
- Coordinate the transfer of patients to other floors or rooms.
- Assist with all ancillary services including dental, optometry, psychological and podiatry services.
- Maintain consistent contact with patients and their support systems to provide comprehensive updates on patient's progress toward goal achievement
- Provides ongoing communication with patients and families regarding discharge needs.
- Maintain confidentiality.
- Ensure a safe and comfortable environment.
- Adhere to the budget guidelines for the department.
- Perform other duties as assigned.
Educational Requirements
- Bachelor’s degree in Social Services or related field (Strongly Preferred).
Qualifications/Skills:
- Minimum 3 years’ experience in working within a skilled nursing, short term rehabilitative environment.
- Knowledge of Federal and State regulations.
- Strong interpersonal skills.
- Excellent verbal, organizational and communication skills.
- Demonstrated critical thinking skills, problem-solving abilities and time management skills.
