Skilled Nursing and Long Term Care Facility in Manchester, NH now interviewing candidates for the position of Social Worker. We are seeking a motivated, qualified Social Worker with excellent leadership skills.
The primary purpose of this job position is to assist in the planning, organizing, developing, of the Social Services Department to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.
Education Requirements and Qualifications:
- Must possess, as a minimum, a Bachelors Degree from an approved school in Social Work or related field.
Major Duties and Responsibilities:
- Assist the Director of Social Services in providing the support and assistive services of the department for patients/residents in the facility.
- Interview residents, family members, as necessary, and in a private setting , to obtain social history
- Involve the resident/family in planning social service programs, objectives and goals for the resident when possible.
- Refer resident/families to appropriate social service agencies when the facility does not offer the services for needs of the resident.
- Provide information to resident/families as to Medicare/Medicaid and other financial assistance programs available to the resident.
- Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of social service programs.
- Participate in discharge planning, development, and implementation of social care plans and resident assessments.
- Maintain an excellent working relationship with the medical profession and other health-related facilities and organizations.
- Coordinate social service activities with other departments as necessary.
- Assure that all progress notes charted are informative and descriptive of the services provided and of the resident’s response to the service.
