Job Description
Title: ASIST Clinician
Department: Outpatient Supervisor: Clinician Supervisor of Bridgeport OP
Status: Exempt
Summary:
- The ASIST Clinician is responsible for conducting clinical integrative assessments and risk and need assessments (when not completed by the referrals source), meeting individually with each client on a regular basis for intensive clinical case management and coordination of services, facilitating Start Now groups and individual and group interventions. Additional responsibilities include client engagement, treatment planning and discharge planning. The ASIST Clinician will provide culturally-responsive, trauma informed, gender-responsive, strength based services, in-person, via telehealth (audio/video), or in hybrid format and will adhere to training, treatment, documentation and programmatic requirements as specified by funders and the agency. The ASIST clinician is also responsible for insuring that all payment authorizations are secured and maintained throughout treatment episodes.
Essential Duties and Responsibilities:
- Maintain flexible schedule to perform clinical assessments and provide individual and group therapy and clinical case management.
- Deliver identified cognitive behavioral interventions.
- Secure initial insurance authorization and all required concurrent authorizations throughout care.
- Provide urinalysis/breathalyzer testing when indicated and within guidelines of monitoring.
- Communicate with referral sources on the status of each client’s progress and performance within the program, including the preparation of reports.
- Complete all written reports and data entry required by JBCSSD, DOC and DMHAS per expected guidelines to support structured communication throughout each client’s entire episode of care.
- Coordinate service provision and criminal justice supervision with other state and local community providers.
- Visit local court at least once a week to education court staff of ASIST services, screen clients and build relationships with referral sources (to include telecommunications and or virtual meetings during times of a pandemic such as COVID-19 or the like).
- Complete all chart documentation according to agency and program standards and funder requirements.
- Input all clinical documentation in Connecticut Renaissance’s EHR accurately and timely.
- Participate in all required internal and external meetings.
- Attend all required trainings to enhance knowledge and skill level
- Comply with licensure and accreditation standards and state and federal regulations.
- Meet agency outcomes measures and participate in action planning around any measures that fall below threshold.
Required Knowledge, Skills, Abilities and Other Characteristics:
Knowledge of:
- Diagnosis and assessment of mental health and co-occurring disorders.
- Fee for service and billing requirements for outpatient services.
- Electronic Health Record
Ability to:
- Communicate effectively both verbally and in writing. Bilingual in English/Spanish preferred.
- Manage priorities effectively to meet deadlines and demonstrate multi-tasking skills.
- Ability to employ effective organizational skills to meet goals
- Able to work independently and as part of a team
Education and Experience:
- Master Degree with Licensure in Behavioral Health (LPC, LCSW, LMFT) in CT preferred. Additional LADC a plus. Minimum required: Associate licensure (LPCa, LMFTa, LMSW)
- Minimum of 3 years of working experience in behavioral health field.
- Experience in cognitive behavioral therapy, motivational interviewing and trauma-informed care preferred.
- Experience with mental health and co-occurring diagnosed clients and criminal justice population preferred.
Other:
- Must be dependable and reliable; detailed oriented; and able to interface easily with clients, staff, and referral sources.
- Must be able to meet routine office physical demands including climbing stairs, carrying up to 20 pound objects and possess the dexterity and vision to operate office computers and equipment.
- Must successfully complete the introductory probationary employment period.
- Valid CT driver’s license.
