Job Brief
Responsible for the development, implementation, and evaluation of individualized treatment plans, provide assistance in independent living skills, activities of daily living, cognitive programming, socialization, recreation, and communication skills.

Responsible for the individual’s well-being.
Manage assigned rooms and other programmatic environments, which include:
Supervision of Habilitation Aides.
Design, implement, and evaluate individual program plans.
Assist people we serve with activities of daily living including toileting, grooming, oral hygiene, meal preparations, and specialized medical needs, as required.
Administer medications and records same.
Develop and participate in special programs/projects.
Develop, monitor, and implement individual treatment plans that are person-centered.
Monitor the people we serve well-being and report any changes in condition and/or health care concerns to nursing staff.
Implement individual behavior plans, as needed, by following prescribed methodologies.
Monitor, prepare, and/or assist in the preparation of lunch/snack and report any dietary concerns to the program director and/or nursing staff. Ensure all mealtime guidelines are being followed.
Actively participate as a team member.
Establish and maintain an effective working relationship with team members.
Participates and may serve as a team leader in the development, implementation, and evaluation of the individual’s program plan.
Design and implement individual treatment plans.
Prepare and monitor the preparation of learning environments prior to each day’s activities.
Maintains and monitors the maintenance of the cleanliness and safety of work environments.
Will notify the Program Director or Coordinator of any safety concerns, immediately upon becoming aware of them.
Facilitation or the monitoring facilitation of special equipment.
Will follow universal precautions.
May conduct and/or participate in fire and evacuation drills.
Provide supervision and programmatic training to Habilitation Aides in the implementation of goals, and ensure proper documentation.
May assume additional supervisory and administrative responsibilities, in the absence of the Program Director or Coordinator, including but not limited to:
Ensuring the appropriate utilization of staff.
Ensuring quality assurance and compliance with governmental regulations, as well as agency policy and procedures.
Ensuring that required documentation is completed, i.e. attendance, incident reports, etc.
May be assigned as a member of a standing committee.
Maintain all requisite documentation, including but not limited to daily data, quarterly, semi-annual, and annual case reviews.
Complete and respond to all requests for information including Incident Reports.
Attend and/or conduct services and specialized training programs and team meetings as assigned.
Demonstrate legal and ethical conduct.
All actions should adhere to those prescribed in the Agency Employee Handbook or Employee Updates.
Perform all other duties assigned.
Requirements:
Education/Experience: BA or BS in appropriate field of study, or experience working with people who have IDD
License:New York State Driver’s License and agency insurability, completion of orientation and medication certification course.
