Job Summary
Provides clinical support and care management for patient populations at high risk for health deterioration and/or poor outcomes.
Responsibilities And Duties
- Provides clinical support for medical practitioners in treatment of high-risk patient populations,
- Assesses health risks unique to each patient
- Educates patients regarding disease process, treatments, and secondary prevention, including lifestyle modifications and proper use of medications and medical equipment
- Monitors treatment adherence and progress as directed
- Serves as clinical contact person, communicating with patients and responding to phone calls and messages
- Assesses health status and coordinates higher level of care when needed
- Schedules follow up appointments
- Arranges and tracks referrals
- Communicates with integrated care team members to ensure the needs of patients are addressed
- Receives calls and messages from patients and coordinates responses
- Relays patient education per procedure, including diabetic/insulin teaching
- Triages patients for walk-in needs and discusses with medical practitioner
- Assists medical assistants in performing technically difficult clinical procedures
- Reads TB skin tests
- Performs nursing procedures
