Requirements

• Massachusetts Licensed Clinical or Independent Social Worker (LCSW or LICSW)
• Current Driver’s License
• Minimum 3 year clinical experience required
• Previous experience working with adult population required
• Previous experience working with geriatric population preferred

Responsibilities

• Provide in-home social service interventions for the stabilization and maintenance of the patient
• Conducts a comprehensive assessment and discharge plan, and documents according to agency and departmental standards and policies
• Identifies and resolves social or emotional problems that impede effective treatment of the patient’s medical condition or rate of recovery
• Designs and documents individualized care plans based on patient need
• Coordinates with agency team and documents interdisciplinary consults and case conferences
• Makes referrals and documents according to agency policy
• Identifies and facilitates resources for patients
