If you’re looking for an exciting and rewarding role, please join our Assertive Community Treatment (ACT) program as a Rehabilitation Specialist. Our award-winning ACT teams are community-based care providers that can create positive and effective change through frequent contact and visits with the members we serve in the community. Staff are required to pivot at a moment’s notice, whether driving a member, assisting adult members, responding to a crisis, or holding group sessions. ACT services reduce crisis and helps members learn skills that are needed to integrate into the community and live independently without support. This evidence-based practice helps improve outcomes for people who are most at-risk of homelessness, psychiatric hospitalization, and institutional recidivism.

Minimum Qualifications:
- Bachelor’s Degree in Psychiatric Rehabilitation, Rehabilitation, Social Services, Psychology, or a related field plus two (2) years of direct care experience OR
- HS Diploma/GED and six (6) years of full-time direct care experience.
- Minimum of one (1) year of experience providing employment/rehabilitation-related services and support to individuals with disabilities required.
- Must have reliable transportation, valid AZ Driver’s License with state minimum insurance coverage and ability to meet and maintain Copa Health’s acceptable driving requirements.
- TB test clearance required at the time of hire.
- Must have a fingerprint clearance card issued by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire
- Bilingual (English/Spanish) speaking preferred.
JOB SUMMARY (Including, but not limited to):
- Provide vocational rehabilitation services, assessments, and support services to our most vulnerable members and serve as the subject matter expert for the team regarding rehabilitation and vocational related services.
- Develop relationships with community employers, providers, or other organizations to foster increased employment and meaningful community activity opportunities for members.
- Engage, educate, communicate, and coordinate care with each member, their family, behavioral and medical providers, community resources, and others to ensure that all services prescribed in each member’s individualized service plan are implemented.
- Collaborates with the ACT members in completing assessments and addressing the needs identified by the assessment.
- Participates in the service plan goal development for each participant as it relates to their rehabilitation goals and the interventions they will need.
- Provides and oversees the provision of any direct skills training(s) as needed.
- Works in close collaboration with the Independent Living Skills Specialist, Housing Specialist, Employment Specialist, and the assigned Vocational Rehabilitation Counselor to ensure coordination of services.
- Completes all clinical documentation in a timely manner in accordance with policies, procedures, and clinical protocols.
- Assures positive member outcomes by providing exceptional member care and timely documentation of services by consistently meeting optimal engagement and performance standards.
- To ensure compliance & adequate services, additional job duties may be required to meet the needs of the program and or department.
