Public Health Nurse

Job Overview
Coshocton Public Health District (CPHD) is seeking a dedicated Public Health Nurse to join our team. The ideal candidate will play a crucial role in promoting and protecting community health through education, advocacy, and public health initiatives. The Public Health Nurse will be assigned to primarily work the Children with Medical Handicaps program.

Responsibilities
Care coordination of the Children with Medical Handicaps (CMH) program; conducting home/ office visits with families; assessing family needs and providing support, information, and referrals as necessary; completing all nursing documentation in the medical record and billing as required by CMH; performing chart reviews and implementing quality improvement measures. Participates in outreach activities to educate clients, providers, agencies, and the community about the CMH program. Actively participates in continuous quality improvement with the agency through individual work plans and/or projects. Participates in activities working toward accreditation status. The individual filling this position will also be trained in basic CPHD-related emergency response procedures and considered essential for any public health emergency response and disaster recovery.

Qualifications
- Current Ohio license to practice as a Registered Nurse (Required)
- Care coordination and home visiting experience (Preferred)

- Must have and maintain a valid Ohio driver's license

- One year experience in public health nursing or equivalent nursing experience (Preferred)
- Cardiopulmonary resuscitation (CPR) certification (American Heart Association or equivalent) (Preferred)
- Excellent communication and interpersonal skills
- Ability to work effectively in a multidisciplinary team environment
