OVERALL OBJECTIVE OF JOB

This professional social services position is assigned to the Outreach Program Unit. This service coordinator will provide a wide array of cross-agency services to students and their families in a variety of settings including the school and the home. Responsibilities while acting as an Outreach Coordinator include addressing all truancy complaints, managing referrals to other community programs designed to decrease or ameliorate truant behavior, and assisting the county school districts and magisterial judges with recommendations for the disposition of these truancy cases. Employment of service coordination skills is important in order that essential information is obtained and utilized to counsel individuals and members of their families. Work is performed in accordance with established regulations, policies, and procedures.

Receives continuous instruction and regular supervision from the Outreach Program Service Coordination Supervisor in regard to daily work duties.

ESSENTIAL FUNCTIONS OF THE JOB
1. Works closely with the assigned school districts’ attendance office to monitor daily student attendance, assesses this data, and provides short-term intervention services for those students displaying truant or other behaviors that impede the success of the student.

2. Works closely with both the student and the families of those identified with truancy problems to provide services on a voluntary basis.

3. Makes continual phone calls and home visits to families participating with the Outreach program to evaluate and adapt any services to ensure continued school attendance.

4. Presents and prepares the School Attendance Improvement Conference (SAIC) with the school and family.

5. Prepares court reports documenting contact attempts and outcomes, attends all scheduled court hearings as a support to the family, and provides information to the Judge in regards to the family’s progress.

6. Keeps logistic and statistical information on truancy cases for quality assurance and service provision improvements. As well as preparing reports as mandated by federal, state, or county policies and procedures.

7. May at times receive referrals from the Student Assistance Program to address truant behavior or other issues that impede a student’s success that do not involve the safety of the child.

8. Will maintain a spot on the Student Assistance Program team at their assigned school as able.

9. Develops positive working relationships with existing community resources, coordinating and negotiating the provision of needed services for identified youth, and provides education to faculty, students, and families regarding services provided.

10. Assures Agency individuals coordinated access to services and provides transportation to Agency individuals when appropriate, utilizing Agency vehicles.

11. Maintains a flexible work schedule that is responsive to the child, family, school and community’s needs providing outreach services in natural settings such as the family home, school, or neighborhood. Work is predominately performed in a school setting and will follow the school calendar, except duties will be performed during the summer as well.

6. Must possess the ability to operate personal computer and related software, to type and utilize other office equipment and to prepare required reports.

7. Must possess ability to make accurate observations and documentation of same in regard to individuals’ needs and make determinations of risk assessment.

8. Must possess the academic knowledge and skill in promoting therapies and social service work and skills with children and youth and families.

9. Must possess knowledge of child development and the ability to conduct assessments and investigations.

10. Must possess ability to provide structured and unstructured life skills instructions and guidance to individuals as needed.

11. Must possess the ability to express empathy and understanding to all individuals.

12. Must be able to interact effectively with children and youth, department staff, counselors, attorneys and the courts.

13. Must have some knowledge of the judicial and court system as it relates to child services.

14. Must have transportation available and a willingness to travel for work-related job duties.

QUALIFICATIONS
Associate’s degree majoring in social welfare field and 3 years of professional casework or human service experience; or Bachelor’s degree majoring in social welfare and 1 year of professional casework or human service experience; or Bachelor’s degree in an unrelated field with at least 12 credits in social welfare and 2 years of professional casework or human service experience; or Bachelor’s degree and successful completion of an accredited Casework Intern Program; or successful completion of a 6-month probationary period as a Service Coordinator I.

CLEARANCES:

· Must work in conjunction with employer to apply for Act 34 PA State Police Criminal History, Act 151 PA Child Abuse and FBI clearances prior to employment. Clearances currently possessed that are less than 4 years old will be accepted until new clearances are available. All clearances must reflect acceptable results.

· Must not appear on Preclusion Lists as defined by “Pennsylvania’s Medical Assistance Bulletin 99-11-05 Provider Screening of Employees and Contractors for Exclusion from Participating in Federal Healthcare Programs.”

· Must successfully complete pre-employment drug screening.
