What are the responsibilities and job description for the Patient Navigator position at Cove Behavioral Health?
SCOPE OF WORK

1. Conduct client administrative and clinical intake into program to include required residential forms, fee agreement and assessment for funding/insurance.
2. Coordinate securing client insurance to include county coverage and eligibility for Intermediate Care Program.
3. Assess and identify barriers for client identification (birth certificate, SS card, ID) and provide resources/support in obtaining these documents.
4. Linkage, delivery and monitoring of primary health care and case management needs to address goals of obtaining stable housing, gainful employment, engagement in behavioral health recovery support and other ancillary services to support these goals
5. Develop transition and discharge plans with clients that include provisions for behavioral health disorders.
6. Coach clients on engagement with treatment plans and self-help recovery services, individualized health and wellness goals, and self-responsibility.
7. Facilitate life skills group with fidelity to evidence-based practices.
8. Completed required data entry and maintain updates of clients into various databases as required by funding sources.
9. Prepare and maintain clinical records in accordance with policy and procedure.
10. Interface positively with community partners.
11. Participate in client case reviews and team meetings.
12. Make referrals to appropriate resource agencies and document outcomes of referrals.
13. Provide crisis intervention, when needed.
14. Assist BHTs to transport clients as required.
15. Other duties as defined by supervisory/management staff.

EDUCATION REQUIREMENTS
- High School - Vocational Training ? Undergraduate Degree in a related field - Master's Degree
CERTIFICATION/LICENSE REQUIREMENTS
- CAP, CAPP or CCJAP
RELATED EXPERIENCE
-Two to Four Years
KNOWLEDGE, SKILLS & ABILITIES
Ability to read and interpret documents such as safety rules, operating instructions, and procedure manuals. Ability to write routine reports. Ability to speak effectively. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, or schedule form. Knowledge of homeless services preferred, 12-step program, behavior modification, case management and addiction.

Compensation: $20.00-$21.63 per hour
Salary : $20 - $22
