Summary:
The Post-Acute Care Navigator works in coordination with the inpatient operations team including physicians, care management, and therapy to ensure timely and appropriate patient care transitions to post-acute care services. This position requires efficient communication across all teams so that patient care is optimized throughout the post-acute care journey.

Responsibilities:
Works with inpatient operations teams to identify appropriate patients for efficient post-acute care transitions.
Develops efficient care transitions for patients needing follow up services.
Identifies opportunity to decrease length of stay in the acute care setting and works with inpatient operations teams to implement strategies.
Builds relationships with internal stakeholders - case managers, acute care therapy, physicians, nursing care leadership and teams to provide expertise in post-acute care communication to patients and families.
Provides reporting trends and activity to appropriate leaders.
Monitors patient satisfaction in targeted service lines and implements tactics to improve and maintain scores.
Responsible for targeted service line growth and quality outcomes.
Attends quarterly meetings with service line leaders for growth initiatives.
Maintains a high level of current knowledge about each Covenant continuum service line and the associated patient populations.
Promote teamwork and direct communication with co-workers and deal discreetly and sensitively with confidential information.
Contribute by identifying problems and seeking solutions.
Promote patient/family satisfaction where possible; participates in departmental efforts to monitor and report customer service.

Other information:
EDUCATION/EXPERIENCE REQUIREMENTS
Associates degree and licensed in the State of Michigan as an RN or an LPN required.
Bachelor’s degree in healthcare preferred.
Experience in post-acute care.
Experience as a liaison or working autonomously.
K NOWLEDGE/SKILLS/ABILITIES
Computer competence, including Microsoft Office, PowerPoint, and Outlook.
Knowledge of standard office equipment.
Experience in clinical area.
Demonstrated knowledge of project management.
Demonstrated understanding of basic communication and customer service principles.
Thorough knowledge of existing Patient Services, Infection Control, Safety, Patient Rights, Legal Issues and Covenant HealthCare policies and the ability to quickly reference policies as needed.
Utilizes effective time management skills.
