Pay rate : $20.50 / hour
$1,500 sign on bonus
Schedule : Monday-Friday 8a-4p or 7 : 30a-3 : 30p 40 hours

JOB DESCRIPTION SUMMARY :
The Team Leader is responsible for day-to-day operations of the community day program. The Team Leader provides quality services and ensures regulatory compliance in the program.
ESSENTIAL FUNCTIONS :
- Provide quality care to program participants in an environment that promotes their rights, dignity, freedom of choice, and individuality.
- Support daily interactions between participants and staff and provide both positive and developmental feedback.
- Ability to maintain multiple schedules with guidance from Assistant Team Leader.
- Train and mentor new staff in all areas including IPPs, personal care requirements in the program, and performance expectations.
- Directly involved with writing the Individual Program Plan for each participant in the program with guidance from the Planning Coordinator / Monitor.
This includes reviewing the plan with the participant and / or the guardian to obtain necessary signatures.
- During the IPP process; clarification of upcoming goals must be determined along with a review of services.
- Involved in planning meetings concerning Individual Program Plans (IPP), and provide support to the participants according to their plan.
Ensure annual signatures are obtained during the annual planning meeting. If unable to attend a planning meeting due to a schedule conflict the planning coordinator may conduct a meeting in your absence.
- Schedule staff to ensure adequate coverage.
- Fully understands and abides by the agency’s code of ethics.
- Fully understands all aspects of participants’ rights and is responsible for promptly reporting any evidence of abuse or violation of participants’ rights.
- Conduct and facilitate team meetings and ensure that open communication is maintained within the community day program staff and address any and all concerns immediately.
- Communicates and interacts effectively and tactfully with all participants, CW team members, family members, guardians, case workers, and other stakeholders.
- Develop activities, curriculum, and recommend program additions to actively promote community outings for participants.
- Complete all necessary reports and submit (when appropriate) to Business Office in a timely manner.
- Transport individuals, as needed, using personal vehicle or agency vehicle (i.e., to appointments, work, etc.)
PHYSICAL WORK TOLERANCES :
- Ability to do standing / sedentary tasks for long periods of time.
- Ability to perform range of motion tasks, including but not limited to; reaching, bending, stretching, stooping and lifting.
- Ability to lift up to 50 pounds (all personnel are trained in correct lifting procedures).
QUALIFICATIONS :
- High School Diploma or GED.
- Previous experience in related field.
- Valid US driver’s license.
- Satisfactory driving record per agency’s insurance company standards.
- Must be able to read, write, and speak English to the extent necessary to perform the essential functions of the job.
- Must be able to perform basic math skills to the extent necessary to perform the job.
- Must have or be willing to obtain trainings within the Agency’s specified timeline including but not limited to : DSP, CPR / FA, Welle.
- Organization / Prioritizing skills.
