Primary Care Nurse Practitioner

Crossroads Treatment Centers was founded on May 5, 2005 to serve a patient population battling with opioid addiction. Since its founding in 2005, we have been a physician led healthcare organization. The company has grown rapidly and now includes 110 treatment centers in 10 states. Each member of the Crossroads team specializes in an area that supports the recovery of over 27,000 patients, monthly. Crossroads' clinical staff includes physicians, pharmacists, counselors, nurses, and other service coordinators. Along with medication management, our staff works closely with each patient to create an individualized treatment plan aimed at building recovery and relapse prevention skills.

POSITION SUMMARY:

We're seeking a fulltime Primary Care Nurse Practitioner to join an office-based Suboxone program for our McKeesport and Greensburg, PA centers and provide Primary Care to our established patient base. The ideal candidate would have the knowledge and skills to provide prevention, screening, intervention and treatment related to substance use disorders

Position Summary: The primary care physician is responsible for providing primary care medical services in accordance with current standards of care and relevant policies and procedures. They will evaluate, diagnose, and management primary care medical issues in the context of substance use disorders.

Essential Duties and Responsibilities include the following and other duties that are assigned.

• Performs medical evaluations, including physical exams.
• Orders labs as clinically appropriate and communicates test results to patients.
• Prescribes appropriate medications and interventions as indicated by patient assessment.
• Coordinates care with other healthcare providers, making referrals when appropriate.
• Stays current on the latest innovations and prevailing medical consensus of treatments for primary care conditions.
• Monitors patients in accordance with medical standards of care and maintains a safe, protective environment for the patients.
• Completes medical documentation according to accepted standards and Crossroads direction.
• Documents any incidents or unusual occurrences per policy and procedure.
• Collaborates with other practitioners, including serving as supervisor for nurse practitioners

Education and/or Experience:

• Licensed and in good standing with the State you are practicing (PA)
• Experience/exposure to substance use disorder treatment preferred
• Must have DEA
• XDEA preferred
