North Shore Registered Nurse

We are looking for a Registered Nurse that is great at caring for patients in the home setting. The RN would be responsible for vitals and head-to-toe assessments and administering medication when needed. Your role involves assessing patient health needs and implementing treatments to ensure optimal patient outcomes. You will collaborate closely with patients, their families, and a multidisciplinary team to deliver personalized care and support the patient's wellbeing in a home setting.

Key Responsibilities:

• Patient Assessment and Care Planning:
• Conduct thorough assessments of patients' health status, including physical, emotional, and psychosocial needs.
• Develop individualized care plans based on assessment findings, patient goals, and physician recommendations.
• Regularly evaluate and update care plans to reflect changes in patient condition or treatment goals.
• Clinical Procedures and Treatment:
• Perform various clinical procedures such as wound care, medication administration, IV therapy, and pain management.
• Educate patients and their families about proper healthcare procedures, medications, and self-care techniques.
• Monitor patient progress and response to treatments, adjusting care plans as needed to promote recovery and independence.
• Patient Advocacy and Support:
• Advocate for patients' rights and preferences in their healthcare decisions.
• Provide emotional support and counseling to patients and their families, addressing concerns and facilitating communication with healthcare providers.
• Coordinate with community resources and healthcare agencies to ensure continuity of care and access to additional services.
• Documentation and Reporting:
• Maintain accurate and detailed patient records, documenting all assessments, interventions, and outcomes.
• Prepare reports and summaries for healthcare providers, ensuring timely communication and coordination of care.
• Compliance and Quality Assurance:
• Adhere to all regulatory standards, policies, and procedures related to home healthcare services.
• Participate in quality assurance activities, including audits and performance improvement initiatives, to enhance patient care outcomes and service delivery.
• Team Collaboration:
• Collaborate effectively with physicians, therapists, social workers, and other healthcare professionals to coordinate care plans and ensure comprehensive support for patients.
• Participate in interdisciplinary team meetings and case conferences to discuss patient progress, goals, and care coordination strategies.

Qualifications:

• Education: Graduate of an accredited school of nursing with current RN licensure.
• Experience: Previous experience in home healthcare, acute care, or a related field preferred.
• Skills: Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal skills. Ability to work independently and manage time effectively.

Working Conditions:

• Schedule: Flexible hours, including evenings and weekends based on patient needs.
• Environment: Work primarily in patients' homes, requiring travel within assigned geographic area.
• Physical Requirements: Ability to lift and move patients, as needed, and perform physical tasks related to patient care.

Pay: $78,000.00 - $86,000.00 per year
