The Family and Community Liaison position emphasizes the Adverse Childhood Experiences prevention program to reduce risk factors while providing long-term support by linking families with coordinated community resources to help them become re-housed and self-sufficient. This role works primarily with parents to determine program service eligibility.

Required qualifications:

  • Have at least a bachelor’s degree in Family Studies, Social Work or a related field and one year experience with family case management or other relevant education, experience or training.
  • Bilingual English / Spanish fluency.
  • Sensitivity to cultural, language, and socioeconomic differences.
  • Have previous experience working with homeless, low-income, or at-risk populations.
  • Be able to relate and communicate effectively with a broad, diverse population that includes fellow staff, parents, and children.
  • Be able to collaborate and work as a team member on behalf of the agency and for the families.
  • Have interpersonal skills such as tact, warmth, and sensitivity.
  • Have administrative skills such as organization and attention to detail.
  • Have basic word processing, data entry, and internet skills.
  • Meet the New Mexico Child Care licensing requirements regarding criminal records background check, hold a valid driver’s license, have car insurance, and be able to travel within the city limits.
  • Be able to provide educational transcripts.
  • Understand mandated reporting for suspected child abuse or neglect.
  • Have a TB test (per childcare licensing requirements).
  • Be able to lift 40 lbs.

Preferred qualifications:

  • Social work licensure (LSW, LMSW, LCSW, etc.).
  • Trauma informed training/certification.
  • Knowledge of local resources.

Responsibilities include, but are not limited to:

  • Carry a family caseload of up to 30 families depending upon enrollment.
  • Conduct Adverse Childhood Experience (ACEs) screening within 45 days of enrollment.
  • Conduct social determinant of health screening (SDOH) withing 45 days of services.
  • Conduct AAPI pre- and post-test with all families.
  • Conduct Family Success Plan (FSP) with each family.
  • Provide and track appropriate referrals to address risks within ACEs.
  • Provide appropriate resources and referrals to link families with a continuum of care to secure housing, food, clothing, transportation, childcare and health care.
  • Ensure all data in recorded and entered for each client.
  • Provide the agency with accurate statistical information about the services and clients the agency serves through regular reporting of data.
  • Utilize electronic record system to record all behavioral health documentation.
  • Develop strong relationships with other community service agencies for the purpose of networking, referrals, and support of the families this agency serves.
  • Participate in monthly meetings with early childhood, family support specialist, and data management staff to compare case notes and compare data.
  • Participate in monthly outreach meetings.
  • Schedule and meet with each family once a week to monitor and review family progress toward goal completion.
  • Meet with direct supervisor at a minimum of two times a month to review work performance and discuss any issues or concerns.
  • Provide thorough, regular communication with agency team members through informal conversation.
  • Attend training workshops and conferences that will benefit the agency or meet personal training needs. Sharing information learned with the agency as appropriate.
  • Treat all family members (adults and children) with dignity and respect.
  • Other duties as assigned related to Family Support Services.

Job Type: Full-time

Pay: $20.00 - $23.00 per hour
