Job Posting for Program Coordinator - Lung Cancer Screening at D-H Medical Center
Overview
In conjunction with the CT Lung Cancer Screening Director, this role is responsible for the development, implementation and coordination of daily operations and workflow for the CT Lung Screening Program. Collaborates with the Director and clerical staff to support all efforts related to guiding patients through the screening process. Acts as a patient liaison for the program, works with referring physician offices and clinics to arrange initial screening and manages follow up testing and all aspects of the CT Lung database including data entry, statistical reporting and program quality assessment.
Responsibilities
Develops, implements and recommends improvements to clinic flow patients.
Triages daily reports for correct imaging order and preliminary eligibility information.
Follows standard process to review/confirm eligibility into program.
Supports, develops and maintains a database for patient tracking, reporting outcomes and measuring quality improvement.
Compiles and maintains participant database, updates e-DH as appropriate and determines and coordinates if external imaging and medical records are required.
Coordinates smoking cessation counseling as necessary.
Counsels patients, makes final screening eligibility determination and communicates results to patients and provider.
Reviews financial aspects of screening and informs D-H providers of participation.
Follows up with patients and providers regarding screening results.
Tracks and coordinates follow ups of positive screens based on radiology interpretation and provider preference.
Develops a process for tracking next steps and provides instruction to clerical support team for scheduling annual screening exams.
Develop and implement patient and provider education.
Performs other duties as required or assigned.
Qualifications
Graduate from an accredited Nursing Program required
Bachelors of Science Degree in Nursing (MSN) with 3 years of clinical experience required.
Proficient with computer applications, including data entry software, word processors, and reporting platforms.
Ability to coordinate and prioritize multiple tasks at once required.
Must have the ability to maintain a high degree of discretion and professionalism, as well as the ability to maintain the confidentiality of sensitive or personal information required.
