The Director, Clinical Transition of Care (TOC) functions as the leader responsible for driving efforts for improved Transfer of Care outcomes. This role is accountable for driving improvements in admission rates and working collaboratively across markets, divisions, DKC and NCC.
Role and Responsibilities:
- Drives clinical outcomes includes implementation, monitoring and reporting
- Identifies and create training opportunities to improve clinical outcomes and increased efficiencies. Coaching and guidance of clinical staff on interacting with Nephrologists, prioritization and time management of tasks and responsibilities.
- Facilitates initial training and support to new Integrated Care Nurses and Care Coordinators in patient care delivery, policies, procedures, and reports.
- Drives workflow efficiencies and accountability with teammates. This includes ensuring RNs working through TOCs, working with IKCA teams on transfer of data and working with discharge planning and hospitalizations, collaborating on reasons and solutions for missed treatments, likely hospitalizations. Drive continuous improvement process to determine root case of hospitalizations for future prevention and readmission
- Collaborates with the Education Team, the Clinical Team, Subject Matter Experts, and Regional Operations Managers on educational tools and policies related to new processes.
- Communicates and works collaboratively across markets, lanes, divisions, companies to drive success. This includes working to create communication channels between care teams and Nephrology practices. Will be adept at creating relationships and using influence with partners across the spectrum.
- Conducts standardized audits of CMs' and CCs' work and patient results. Provides and/or monitors monthly scorecards and other related reports and creates action plans for each CM and CC.
- Leads and provides critical input to TOC metrics. Accountable for metrics such as TOC and readmit rates.
- Other duties as assigned.
- TRAVEL: Approximately 25% up to 50%.
- NIGHTS/WEEKENDS: Very Infrequent (but as required)
Requires Education and Experience:
- Min of 10 years Relevant Work Experience
- Bachelor's degree in Nursing
- Current RN license in state of practice
- Minimum of ten years' clinical experience in case management and/or renal services
- Minimum of three years' management and/or administrative experience
- Minimum of two years' project management and/or project implementation experience
- Intermediate proficiency in MS Word, Excel, PowerPoint, & Outlook
- Prior experience managing remote teammates

Salary/ Wage Range
$100,100.00-$147,000.00 / yearCompensation for the role will depend on a number of factors, including a candidate's qualifications, skills, competencies and experience and may fall outside of the range shown.
