What are the responsibilities and job description for the Nurse Home Visitor position at Default Brand?
Southwest Human Development is seeking two Registered Nurses to be a Nurse Home Visitors serving first-time pregnant mothers in the North Phoenix and East Valley areas through the Nurse Family Partnership (NFP) program.
The NFP program is an evidenced-based preventative intervention for first time families using a therapeutic, nurturing, and client-centered approach. The nurse uses their expertise to assess the status of the family and provides support through teaching and providing the appropriate referrals to optimize achievement of personalized goals. This program focuses on helping young women live healthier, have healthier babies, and be successful parents.
Primary Duties:
- Provide comprehensive community health nursing services to women and their families eligible for the NFP Program.
- Maintain the highest standards in clinical nursing practice and adherence to the NFP model, and to policies, procedures, guidelines and standards of NFP.
- Provide preventative health education and developmental guidance to families.
- Participate in agency and statewide training and planning meetings.
- Responsible for scheduling appointments with families and coordinating services in the community and with other state agencies as necessary.
- Participate in coordination of care with interdepartmental and interdepartmental teams.
- Responsible for detailed record keeping of client files and weekly reporting of activities.
Qualifications:
- Bachelor of Science Nursing degree is required.
- Must have a current Registered Nurse (RN) license through the Arizona State Board of Nursing, or be license eligible.
- A minimum of one (1) year of nursing practice / pediatric experience is required, three (3) years is preferred.
- Experience in maternal / child health and/or community health nursing and home visiting is highly desired.
- Knowledge of community resources in areas of health, nutrition, child development, child welfare and developmental disabilities is desired.
- Valid Arizona Fingerprint Clearance Card – OR – must qualify for a valid Arizona Level One Fingerprint Clearance Card.
- Must have a current Arizona driver’s license and an insured, reliable vehicle.
- Bilingual (English/Spanish) is preferred, but not required.
