SUMMARY
The licensed social worker is responsible for providing assessment and therapeutic services in his/her licensed profession for assigned individuals.
MINIMUM QUALIFICATIONS
- Licensed Clinical Social Work degree from an accredited institution and current New York State licensure in Social Work required
- One-year full time treatment experience with persons having developmental disabilities, preferred
- NYS driver’s license that meets agency guidelines
- Basic computer skills
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Act in the capacity of a professional practitioner to provide diagnostic, palliative, preventative or habilitative/rehabilitative services to eligible persons
- Provide services in accordance with generally acceptable community standards of professional practice and in conformance to requirements of all-applicable statutes and regulations. This includes but is not limited to 14 NYCRR Parts 679, 620, 624, 633, 635, and 636
- Document in each person’s confidential clinical record all diagnostic assessment reports, individual plans of treatment and treatment notes regarding service being recommended and/or delivered
- Per treatment recommendations, consult with each person receiving services and/or collaterals regarding treatment outcomes and course of treatments
- Serve as a resource to all staff and consumers on a formal and informal basis
- Develop a working knowledge of relevant OPWDD and SARC policies and procedures
- Commit to working with, and supporting individuals served by Schenectady ARC to help them be aware of their potential and to help them achieve their goals and ambitions
- Comply with Agency Corporate Compliance Policies and HIPAA Regulations
- Report all incidents of suspected rights violation, mistreatment, abuse, neglect or exploitation of individuals served
- Attend in-service training, and other professional development as required
- Participate in agency committees as assigned
- Ensure a safe working environment by following all safety guidelines and immediately reporting safety hazards to supervisor
- Report all on the job injuries to Supervisor within 24 hours of the occurrence and complete accident report
- Check agency email on a regular basis to stay informed of important agency communications
- Uphold the Agency mission and vision statements
- Other duties as assigned
